General Information
Description of Service: Offers short-term, transitional housing and support to persons in need. Three homes are available. All utilities are paid and residents may stay up to 4-6 months. Specific rules and regulations set up by the governing committee must be followed. A case worker visits with the residents weekly to monitor progress toward self-sufficiency. Resident must also be willing to commit to program goals.

Intake Procedure: Call 2-1-1 for a screening and referral to CHART for a shelter bed assignment
Intake Requirements: Income & household member verification, bill documentation, emergency contacts
