Description of Program
Provides free medical and dental care to uninsured individuals whose total household income is no greater than 250% of the federal poverty level. Medical services include chronic disease management, health screenings, medication therapy management. Specialty care includes endocrinology, gynecology, ophthalmology, physical therapy and rheumatology. Dental services include cleanings, fillings, and selected restorative care.
