Description of Program
Provides funds for services through a contracted provider to help individuals with a traumatic brain injury (TBI) live independently in their homes and communities. Services include:
Service plan development
Services are provided in residential facilities, day facilities (outpatient), and other home- and community-based settings.

To enroll call 717-772-2762 or 1-866-412-4755. After answering a few simple questions, the Enrollment Specialist will mail a Head Injury Program application to you. You can also expect to be contacted by a Pre-enrollment Assistance Coordinator who will offer you individualized support in completing the application as well as assist in identifying any other statewide resources.
Enrolled individuals may receive no more than 12 consecutive months or $100,000 for rehabilitation services as specified in the rehabilitation service plan, followed by a maximum of six consecutive months or $1,000 for case management to assist in transitioning out of the HIP. Certain situations may cause an individual's enrollment period to end prior to the time designated in the rehabilitation plan.
