General Information
Description of ServicePatient Advocate Foundation Co-Pay Relief Program provides direct financial assistance to qualified patients, including Medicare Part D beneficiaries, assisting with co-payments, co-insurance and deductibles required by your insurer for pharmaceutical treatments and/or prescription medications relative to your diagnosis. Co-Pay Relief call counselors work directly with the patient as well as with the provider of care to obtain necessary medical, insurance and income information to advance the application in an expeditious manner. Upon approval, payments are made:
  • To the Doctor,
  • To the Pharmacy,
  • To the Patient Directly.

EligibilityEligibility requirements include:
  • Patient should be insured and insurance must cover the medication for which patient seeks assistance,
  • Patient must have a confirmed diagnosis of the disease/illness for which they seek financial assistance,
  • Patient must reside and receive treatment in the United States,
  • Patient's income must fall below 400% of the Federal Poverty Guideline (FPG) with consideration of the Cost of Living Index (COLI) and the number in the household.
