Description of Service
A program of tenant-based rental assistance coordinated with either Blended Case Management or Housing Support Services, for fixed funding intervals based on Priority Categories of Need.
Applications for assistance are accepted during open enrollment periods, approximately 4 times per year. Emergency applications are accepted at any time.
Applications must be completed and submitted with one of the following: a Blended Mental Health (or) Drug and Alcohol Case Manager, a Mental Health Treatment provider, or a Substance Abuse Treatment provider.
During open enrollment periods, applications are reviewed and approved according to specific Priority Categories of Need. These are:
*homeless- street or place not meant for human habitation
*state hospital discharge
*Reading Hospital or Berks Counseling Center ACT Team participant
*transitional housing/group home discharge
*either diversion form or release from Prison System, within past 60 days
*transitional aged youth (18-21), transitioning from CYS or JPO Systems including residential or foster care placement
*hardship- paying more than 50% of gross monthly income towards rent and basic utilities and under a current, official eviction
*rooming house resident
*lottery- paying more than 50% of gross monthly income toward rent and basic utilities (approved lottery applications are included into a lottery "draw" for random selection at the end of each open enrollment period
Emergency applications are accepted at any time, for consideration of assistance in the following circumstances:
*acceptance into Section 8 or other private subsidized housing, with assistance for security deposit payment as requested
*utility assistance requested
*foreclosure/tax sale of rental property
*placarded property by Codes/Zoning
*fire or natural disaster
*other circumstances of eviction unrelated to non-payment of rent
Must be at least 18 years old, must be able to be named on a lease as a responsible party, must have CCBH (Medical Assistance) Insurance, must have a diagnosis of a serious, persistent mental illness and/or substance abuse, must have some source of verifiable income within the household.
