General Information
Description of ServiceThe VA Pittsburgh Healthcare System Heinz Campus’ Community Transition Center (CTC) is an outpatient program that provides Veterans with supportive services and skill-building courses addressing individual needs through small group and one-on-one sessions.

The program was developed to promote independence, hope, empowerment, and confidence in one's ability to manage health concerns while attaining a more fulfilling life in the community.

Initial criteria for participation in this program is VA enrollment with a primary diagnosis of serious mental illness and functioning at an inadequate level in health needs, social interaction, daily living, or community resources.

The program is located at the Heinz campus in the Veterans Recovery Center. Skill courses run on a quarterly basis and are offered five days a week, with veterans utilizing the program by participating in groups directly related to their identified recovery goals.

  • Mental Health in Recovery,
  • Spirituality,
  • Stress Management,
  • Discovering Your Interests,
  • Money Management,
  • and more.

  • Services are designed to assist veterans in developing the skills needed to inspire hope, confidence and the ability to succeed in managing health concerns while attaining a more fulfilling life in the community. CTC groups have also assisted veterans that struggle with job-related and/or legal problems due to symptoms of their mental illness.
