Description of Service
The HJ Heinz Community Living Center (CLC) Social Work Program provides comprehensive social work interventions to those patients and caregivers within the Community Living Center setting including: long-term care, short-term rehabilitation, dementia specialty care and hospice.
Services include: comprehensive assessment and discharge planning, patient/caregiver education, psychosocial support, crisis intervention, grief counseling, and assistance with medical decision-making. Social Work facilitates referrals to both VA and community resources to maximize post-discharge support for patient and caregivers. Social Work also provides a wide range of clinical support services within respite programs.
The H.J. Heinz Social Work Program also works closely with the outpatient, behavioral health and acute care social work programs to facilitate a smooth transition for veterans moving within various levels of care to more fully meet their care needs.
Monday through Friday, 8:00am to 4:30pm
General eligibility requirements for long-term care admission to the CLC include: 60% service connection with unemployability or higher; requires skilled nursing care for a service related disability/ illness; or requires a skilled nursing level of care to meet medical or ADL/IADL needs for short term duration.
