Description of Service
VA Pittsburgh Healthcare System's University Drive Social Work Program provides comprehensive social work interventions to those patients and caregivers within the acute care setting including: medical, surgical, intensive care units, and the emergency department.
Services include: comprehensive assessment and discharge planning, patient /caregiver education, psychosocial support, crisis intervention, grief counseling, and assistance with medical decision making.
Social Work Facilitates referrals to both VA and community resources to maximize post discharge support for patient and caregivers. Social Work also provides a wide range of clinical support services within the Same Day Procedures Unit, 23 Hour Observation Unit, Rainbow Team, Radiation/Oncology, Infectious Disease, Spinal Cord Injury, Renal, GEM, the Transplant Program, Primary Care, Physical Medicine and Rehabilitation Program, VIST/Low Vision Program, and within inpatient and outpatient behavioral health programs.
Monday through Friday, 8:00am to 4:30pm
