Description of Service
VA Pittsburgh Healthcare System's Contract Community Nursing Home Program (CNH) coordinates nursing home placement in community nursing homes for qualifying Veterans.
Participation in the Contract Community Nursing Home Program requires that a veteran be admitted to a VA contract nursing home. This program falls under Veterans Millennium Health Care Law 106-117: “Community Nursing Home care may be provided to veterans who are in need of such care for a service-connected disability and have a service-connected disability rated 60% or more.” If a veteran does not meet the above criteria, a social worker can determine if funding is available for a short-term contract. Veterans under CNH contract are visited every 30 days by a CNH nurse or social worker. The CNH team assists with discharge planning, coordination of VA follow-up appointments, and overall monitoring of the quality of care provided in the contract nursing home. In addition, contracted facilities undergo a yearly review to ensure that they continue to meet VA quality standards.
