General Information
Description of ServiceVA Pittsburgh Healthcare System's Home Telehealth program provides chronic disease management through the daily use of Telehealth technologies.

Veterans diagnosed with chronic diseases (such as diabetes, hypertension, kidney disease, heart failure, chronic obstructive pulmonary disease, post-traumatic stress disorder, depression, and substance use disorder) who meet enrollment criteria receive a home monitoring device which transmits information such as blood pressure, weight, glucose readings, and mood to a Home Telehealth nurse s that medical and psychiatric conditions can be closely monitored. Veterans can participate by using a land line telephone, their personal cell phone, the internet, or via a cellular modem provided by VA. The Home Telehealth program improves communication with the veteran and medical team to improve symptom management and decrease emergency room visits, hospital admissions and long-term nursing home placement.

Criteria for enrollment includes:
  • Enrollment in VA Pittsburgh’s primary care program.
  • Living within a two-hour drive of VA Pittsburgh Healthcare System,
  • A chronic disease for which the primary care provider feels home Telehealth would improve clinical outcomes and resource utilization,
  • Multiple hospital admissions, frequent phone contacts, or multiple primary care clinic visits,
  • Multiple prescriptions,
  • Travel issues, such as geographic isolation, rural home or home bound,
  • Home environment that supports managing daily care and medical problems at home (access to utilities and safety concerns are addressed for appropriate installation of equipment),
  • Acceptance of technology in the home,
  • Demonstrated competency in using and maintaining Telehealth equipment,
  • No history of behaviors that many jeopardize the safety of staff or equipment.
The VA Home Telehealth Program consists of nurses, health technicians, and a program support assistant that work together closely to ensure that Veterans receive the right care in the right place at the right time.
