General Information
Description of ServiceMilitary Pathways is a Department of Defense website where you can take an anonymous mental health self-assessment test.

If you represent a military unit, installation or DoD/VA organization that supports military members, veterans or their families you can also order program materials.

The Department of Defense is offering the Military Pathways program as part of its ongoing support of military members and their families. The program is accessible online at http://www.militarymentalhealth.org. It provides free, anonymous mental health and alcohol self-assessments for veterans and family members and service personnel in all branches including the National Guard and Reserve.

The Department of Defense and National Center for Telehealth & Technology (T2.health.mil) provide these materials free of charge to support your military community. Ordering does not guarantee receipt of a product, as the demand is often greater than the inventory. Please provide as much detail as possible regarding your use of the products to help ensure receipt. Priority is given to Military Installations.

We will contact you to confirm your order prior to sending it. Additionally, please be aware that feedback is required from you about how you utilized the materials if you receive a kit. You must complete this information within two weeks of using the kit. Failure to provide feedback may impact your ability to receive Military Pathways materials in the future.

Military Pathways will no longer provide paper screening forms as part of our kits. If you would like to use paper screening forms, you may make copies from the Military Pathways Downloadable Materials page.
