Description of Service
The HCHP provides access to comprehensive health services through clinics at homeless congregate sites for homeless men, women and children. Health teams comprised of physicians, nurses, nurse practioners, case managers, and outreach workers staff weekly clinics throughout the Pittsburgh area. Some of the services given by the medical team include needs assessment, case management, health education, gynecology, dermatology, podiatry, dentistry, optometry and referrals for additional services. A prescription voucher program is also available to homeless individuals who have no medical assistance for covering prescription costs. Additional specialty services, outreach services, and substance abuse services are available to homeless individuals each month. Such services include podiatry, OB/GYN, pediatrics, case management, AIDS/HIV testing/counseling, and drug and alcohol abuse services.
Primarily Allegheny County.
Call main office. Services provided at each clinic may differ and clinics do not take appointments.
Homeless persons. This includes those who are street homeless, doubled up, or in a shelter. It could also include those in transitional housing depending on the clients situation prior to their placement in transitional housing.
This program is not able to assist those who are wardens of the state.
