Description of Service
Office of Intellectual Disability provides intake and registration for supports coordination to eligible individuals with intellectual disability and / or autism.
Registering for supports coordination and future services begins with a call to OID. An over-the-phone questionnaire will be completed to begin the registration process. If eligible, an intake meeting will be scheduled.
Upon completion of registration individuals will choose a Supports Coordination Organization (SCO). A Supports Coordinator from that SCO will assist in developing a plan for supports and services.
Questions can be answered about available services and supports and regarding assistance for families of children 0-9 with developmental disability.
