    Veterans Health Administration, VA Pittsburgh Healthcare System - Suicide Prevention ProgramVA Pittsburgh Healthcare System's Suicide Prevention Program serves to facilitate implementation of suicide prevention strategies throughout VAPHS with the goal of raising awareness and prevention efforts throughout the system as guided by VA Central Office (VACO). The Suicide Prevention Team consists of social workers (a coordinator and case manager), who strive to achieve this goal through a variety of means. Outreach activities that provide education to veterans, families, and providers both inside VAPHS and I the surrounding community are completed monthly by the team to increase awareness about suicide risk factors and warning signs, as well as available treatment options to those in need. The team provides psychoeducatioanl groups to assist VAPHS veterans. Program staff works collaboratively with VA Crisis Line and VA Homeless Hotline to assist in providing follow up to crisis calls, and in many cases, assist with accessing care. Suicide Prevention is also tasked with providing regular case management to those veterans determined to be a high risk for suicide, providing assistance with access to care and monitoring their well-being during the period of highest risk (30 days after behavioral health hospitalization).     Veterans Health Administration, VA Pittsburgh Healthcare System - University Drive Campus: Home Based Primary CareVA Pittsburgh Healthcare System's Home Based Primary Care program (HBPC) provides primary and tailored health care and skilled nursing services for the home bound, chronically ill, and frail elderly Veteran population within Allegheny County The HBPC interdisciplinary team provides medical and psychiatric care, nurse case management, clinical social work services, rehabilitation services, dietician and pharmacy services. All interventions are designed to meet the Veterans' individual needs in order to allow them to remain in their homes and manage their chronic disease processes. Veterans who cannot safely live alone must have a caregiver who is willing and available to assist in their care. HBPC program provides:
