HealthWell FoundationThe HealthWell Foundation provides financial assistance to eligible individuals to cover coinsurance, copayments, health care premiums and deductibles for certain medications and therapies. If you've been prescribed a medication and your insurance company covers it, but you still cannot afford the coinsurance or copayment required, we may be able to assist you by paying for part of your costs associated with the medication. Also, if you are eligible for health insurance, but cannot afford the insurance premium, we may be able to assist with your insurance premium. Please keep in mind that the HealthWell Foundation currently does not assist with the following expenses:
  • Office visits,
  • Administration or infusion costs,
  • Medical equipment and supplies,
  • Medical transportation,
  • Health insurance premium costs for an entire family,
  • Deductibles that apply to treatments and services that are not eligible for your approved disease fund,
  • Pharmacy compounding costs,
  • Over-the-counter treatments,
  • Treatments for side effects (e.g. nausea).

Links to IndependenceHome-based habilitation - Assistance & training are provided to improve the skills necessary for the individual to live as independently as possiblle. Respite Services - respite periods afford even the most dedicated parents & family members needed breaks. The individual remains in their own home with the support of the staff member and is able to continue with theri typical daily activities. Transportation for activities provided by a staff member. Personal care services provided when necessary either in the home or community, while strengthening the abilities of the individual to care for themselves. Patient Advocate FoundationPatient Advocate Foundation's Patient Services provides patients with arbitration, mediation and negotiation to settle issues with access to care, medical debt, and job retention related to their illness. Patient Services may assist you with the following services:
  • Co-Pay Relief Program;
  • Outreach Programs;
  • Prevention assistance;
  • Case Management Assistance that focuses on Access to Care Related Assistance, Financial Assistance, Insurance Assistance, and Job Retention Assistance.

Patient Advocate Foundation - Co-Pay Relief ProgramPatient Advocate Foundation Co-Pay Relief Program provides direct financial assistance to qualified patients, including Medicare Part D beneficiaries, assisting with co-payments, co-insurance and deductibles required by your insurer for pharmaceutical treatments and/or prescription medications relative to your diagnosis. Co-Pay Relief call counselors work directly with the patient as well as with the provider of care to obtain necessary medical, insurance and income information to advance the application in an expeditious manner. Upon approval, payments are made:
  • To the Doctor,
  • To the Pharmacy,
  • To the Patient Directly.
