    Programs that offer an interactive decision support process that helps individuals in need of long term care and their families understand their strengths, needs, preferences and unique circumstances and weigh the pros and cons of available alternatives. The consultation includes a discussion of the factors to consider when making long term care decisions, information about the range of long term care support options available in their community (such as personal care, transportation and medication management) and resources that can help them pay for services. The program also provides decision support to help identify next steps in the process and help in connecting with services, if needed. The service is generally available to older adults and adults of any age who have a disability; can be of benefit to people using their own resources to pay for services; and may be provided over the telephone or in person (at home, at an agency, in a hospital, at a rehabilitation or nursing facility or in another setting of the person's choosing). The objective of the program is to allow people to live as independently as possible in the setting of their choice.

    Allegheny County DHS - Nursing Home TransitionOffers informational and counseling sessions at Allegheny and Butler County nursing homes to make long-term care consumers aware of community-based care options. In cases where relocation back into the community is appropriate and desirable for the consumer, NHT staff work closely with the consumer, family caregivers and nursing home staff to provide the necessary resources and assistance throughout the transition process. A state-wide initiative of the Pennsylvania Department of Aging to expand the range of long-term care opportunities beyond traditional nursing home care for both elderly consumers and consumers of all ages with disabilities.     Fulton County Medical Center - Long Term CareFulton County Medical Center is fortunate to have a Nursing Home as part of the community. We fulfill this mission by providing a home to 67 residents in our long term care unit on the FCMC campus. What makes us unique from other nursing homes is that we have easy access to many in-house services. The Activity Department at the Fulton County Medical Center offers a wide variety of programs. Its mission is to value each resident and engage them to their fullest potential by participating in programs of their interests, while also educating, encouraging and expanding their horizons. The residents have a choice to participate in attending social, recreational, and cultural events. On the campus we offer such activities as baking groups, craft groups and sing alongs, to board games, wii games and bowling.
